Scoping Review: mapping a field before reviewing it

A scoping review answers a different kind of question than a systematic review: not “does it work?” but “what is out there?” — what has been studied, with which methods, on which populations, with which concepts, and where the gaps sit. It keeps the systematic machinery (defined criteria, documented search, transparent screening) but replaces quality-weighted synthesis with charting: mapping the studies onto the dimensions that matter. Done well, it is the fastest way to make an unfamiliar literature navigable. Done lazily, it is an annotated bibliography wearing a methods section.

If you find yourself wanting to conclude that an intervention works, you have outgrown the scoping review — the design does not license that claim, because it never weighed the evidence.

Origins and the framework everyone cites

The method was formalised by Arksey and O’Malley in Scoping studies: towards a methodological framework, which named the five stages still in use: identify the question, identify relevant studies, select studies, chart the data, and collate, summarise and report.

Levac, Colquhoun and O’Brien tightened each stage — team-based screening, iterative criteria, an analytic rather than descriptive charting step — in Scoping studies: advancing the methodology.

Reporting has its own standard: PRISMA-ScR, published by Tricco and colleagues as PRISMA Extension for Scoping Reviews. Most journals that publish scoping reviews now expect the checklist.

The five stages, with the upgrades that matter

Define the question — broadly, but with anchors

Scoping questions are wide by design (“what is known about peer support in youth mental health services?”), but they still need a stated population, concept, and context — the PCC frame — or the search cannot be built.

Write down what would make a study relevant. You will revise it; the revision history is part of the method.

Search like a systematic review

The searching discipline is identical: multiple databases, recorded strings and dates, grey literature where the field lives outside journals. Breadth of question is not a licence for a casual search.

Select with iterative criteria

Criteria may be refined as screening reveals the field’s actual shape — that is legitimate and distinctive to scoping reviews — but each refinement is dated and reported. Two screeners on at least a sample keeps the selection honest.

Chart the data analytically

Charting is extraction re-purposed for mapping: year, country, design, population, concept, outcomes measured, key findings. The Levac upgrade is to treat charting as analysis — choose dimensions that answer the review question, not just ones that are easy to fill.

This is where scoping reviews go soft. A chart with only bibliographic fields produces a review that can count studies but cannot characterise them.

Report the map, name the gaps

Summarise numerically (how many studies, of what kinds, where, when) and thematically (what the field asks and finds), then state the gaps as specifically as the map allows: not “more research is needed” but “no study has followed participants past six months, and none was run in a low-income setting”.

Common mistakes

  • Running a scoping review to smuggle in an effectiveness conclusion. The design cannot support it; reviewers know this.
  • Charting only bibliographic trivia. Year and country tell a reader nothing they could not get from the reference list.
  • Treating iterative criteria as no criteria. Refinement is documented evolution, not improvisation.
  • Skipping PRISMA-ScR. The checklist exists; omitting it reads as not knowing it exists.
  • Appraising quality half-heartedly. Either appraise with a named tool and use the result, or state clearly that appraisal was out of scope and why — the halfway version satisfies nobody.

Limitations

A scoping review inherits the visibility biases of publication: what is mapped is what got published, indexed, and written in the searched languages. The map is of the literature, not of the practice.

Because quality is usually not appraised, the map weights a weak study and a strong one identically — fine for “what exists”, misleading the moment counts get read as evidence.

And breadth costs depth: 87 charted studies cannot each be read closely. The method trades interpretive depth for coverage, which is exactly why it pairs well with a follow-on systematic review of the densest cluster.
